Live Scan - Fingerprinting Appointment Request Form
This the OLG Live Scan Appointment Request Form to get fingerprinted with the Archdiocese of Los Angeles (ADLA).
If you've been fingerprinted elsewhere that is not the Archdiocese of Los Angeles (i.e. public school, law enforcement, military, etc...), then you are not in ADLA system. The law does not allow different entities to share this information. Therefore, you will be considered as a first-time applicant for ADLA and need to get fingerprinted.
2) If you were a volunteer and is now an employee or coach, you will need to get fingerprinted again.
3) If you think you've been fingerprinted with ADLA, please give Jamie Ramirez a call to look you up in the system, 310.372.7077 ext.228.
This is free of cost. Driver License / official ID is required at your fingerprinting appointment.
There is often a wait list. Please let us know if you need to cancel your appointment. Someone on the waitlist will be most appreciative of you giving up your spot.
